I was a patient for a few months and was mislead lied to and not fully cared for. The billing and administration at this facility is horrible and so money hungry. I had many bad experiences with a few nurses Brenda being one of them and staff. I didnt feel safe at times. Got alot of runarounds and instead of recovering I was constantly stressing out. They made to many promises in the beginning and many were not completed in a timely manner. There events director Candice is mean and rude. She came into my room before Christmas yelling at me and accusing me of something I didnt do. The case worker I had never did nothing for me during my stay or when being discharged. I was sent home with no way to get around. She locked herself in her office never returning calls or visiting with me as needed. Till this day I’m having a hard time getting things resolved thanks to them. The only department that was good and didn’t give me trouble was a few good nurses, aids and the therapist in the therapy department. Joe one of the administrators before I left never came through with what he needed to do. To many broken promises. If you want good care for you and your loved ones. I dont recommend this facility.

Not a nursing home, it is a rehab/ physical therapy center/after care. My mom was in the hospital for 2 weeks, then came here to try and get her up and moving again. Staff treated her well… there was a sitter outside her room but looking after a few rooms. Rooms are clean and spacious, the facility is huge so they have a lot of people to look after. Since my mom needed monitoring, she was in a different wing. Staff is patient but stern. I felt bad for the staff when they would report to me that my mom hit them, but they would tell me she’s just frustrated and they are trained to handle those situations with grace and dignity. Meals are served on time, they have activities, a nice courtyard, eating area, families are welcome to visit and join some activities or just be with their loved one. If there were any changes with my mom, I was always informed. Thank you to all the staff at Canyon Vista!
The care my mother has received has been very good. The facility is nicely maintained and most staff a very friendly. The one thing that needs improvement is communication with family. A contact person should be identified upon admission. I have felt like I have to try and chase people down to get updates. Relying on my mom to report what’s happening with her care has been challenging. Therfore, direct staff contact would be helpful.
If I could give this place a zero star I would right now. My dad is there for rehab after quadruple heart bypass. He has 2 bedsores is continuing skipped for therapy. How can he possibly get stronger if they just leave him laying in the bed. They put a “wound care mattress” on his bed which is doing nothing clearly because he’s got bedsores form not being moved. I am highly disappointed that this is the facility I picked for his care. I’m expecting a turnaround I. Care immediately. Beyond mad 🤬🤬🤬

Visited my B-I-L many times over 2 months. Much better than previous facility. He liked the food choices, and said his care was excellent! Staff were always friendly and helpful to us.

I’ve had several talks with family members who have relatives there. It’s not a good place. All the posts say this place is so very good; then why do they rate only a 3.5? Not a good place. They do nothing for the poor elderly people who have the misfortune to be placed there. They just leave them there in their beds to die. So sad. So shameful.

My brother was brought over there this past weekend. To start things off his feeding tube was accidentally removed. Since then all he’s gotten is IV fluids. Also when putting on the nurses light there is no sound.But there not at the nurses station to see the light go off on the board. Instead there’s 5-6 people eating lunch at the next n nurses station. So only if there willing to look for lights on,the patients lay in there urine & poop for over an hour. When asking the CNA’S for help during lunch,your told after there done handing out the lunch trays. They keep on saying they want to do a swallow test all week,now we’re being told Monday & also Friday he’s being taken to have his feeding tube replaced. Meanwhile nothing by mouth all week long. When you need a nurse or cna good luck. You can walk the halls n never find one. This place is looking like the skilled nursing on rainbow n Russell.As of today he’s still NPO nothing by mouth. The feeding tube hasn’t been pu t on nor will they give him PAIN MEDS BY MOUTH.Would be nice if the owner contacted us if there wanting to know how the facility is being run.Also staffing walks around with street clothes so you dont know if there a doctor ,nurse ,cna or a visitor. A lot of work needs to be improved for me to change my review. At the moment still dealing with the same issues. Waited today over an hour today for the nurse Florida to put in an IV . Waiting on Xrays & ct scan to see if he has a blockage. Meanwhile the appeal only last til 3/5

After 90 days at canyon Vista Post Acute rehab hospital, they failed in Teaching me to walk. After transferring to a group home, the chief caregiver was able to help me walk within a few weeks. The only notable things I got from canyon Vista post-acute were covid and MRSA (Methicillin-resistant Staphylococcus aureus), also known as l foolish eating bacteria. 2 years later I am still fighting with it. I had asked for a COVID shot and they said they didn’t do that kind of thing. And then I got COVID and spend over a week getting over it. People die from COVID. I will probably die from the MRSA.

Not a good experience signing in as a visitor, very tacky and annoying check in machine that experience was not fun but the facility is nice just poor tech
