These kids are very good
Visited my B-I-L many times over 2 months. Much better than previous facility. He liked the food choices, and said his care was excellent! Staff were always friendly and helpful to us.
Very nice place and the receptionist on the weekends is nice
I’ve had several talks with family members who have relatives there. It’s not a good place. All the posts say this place is so very good; then why do they rate only a 3.5? Not a good place. They do nothing for the poor elderly people who have the misfortune to be placed there. They just leave them there in their beds to die. So sad. So shameful.

My brother was brought over there this past weekend. To start things off his feeding tube was accidentally removed. Since then all he’s gotten is IV fluids. Also when putting on the nurses light there is no sound.But there not at the nurses station to see the light go off on the board. Instead there’s 5-6 people eating lunch at the next n nurses station. So only if there willing to look for lights on,the patients lay in there urine & poop for over an hour. When asking the CNA’S for help during lunch,your told after there done handing out the lunch trays. They keep on saying they want to do a swallow test all week,now we’re being told Monday & also Friday he’s being taken to have his feeding tube replaced. Meanwhile nothing by mouth all week long. When you need a nurse or cna good luck. You can walk the halls n never find one. This place is looking like the skilled nursing on rainbow n Russell.As of today he’s still NPO nothing by mouth. The feeding tube hasn’t been pu t on nor will they give him PAIN MEDS BY MOUTH.Would be nice if the owner contacted us if there wanting to know how the facility is being run.Also staffing walks around with street clothes so you dont know if there a doctor ,nurse ,cna or a visitor. A lot of work needs to be improved for me to change my review. At the moment still dealing with the same issues. Waited today over an hour today for the nurse Florida to put in an IV . Waiting on Xrays & ct scan to see if he has a blockage. Meanwhile the appeal only last til 3/5

After 90 days at canyon Vista Post Acute rehab hospital, they failed in Teaching me to walk. After transferring to a group home, the chief caregiver was able to help me walk within a few weeks. The only notable things I got from canyon Vista post-acute were covid and MRSA (Methicillin-resistant Staphylococcus aureus), also known as l foolish eating bacteria. 2 years later I am still fighting with it. I had asked for a COVID shot and they said they didn’t do that kind of thing. And then I got COVID and spend over a week getting over it. People die from COVID. I will probably die from the MRSA.

Not a good experience signing in as a visitor, very tacky and annoying check in machine that experience was not fun but the facility is nice just poor tech

Definitely recommend this facility. Great staff, great location, and great care.
I am currently a patient of the facility. The staff is very friendly and they are addressing all of my needs. The facility is nice and clean. If someone ask me, I would give them my personal approval for the facility.
My husband has been in this facility going 2 weeks. It is so hard to get a Nurse or CNA to come to his room for anything. I should not have to go looking them. CNA’s come in to change him and clean him up. I have heard one excuse after another. One said to me we need to hurry because it was his lunch time. I will have to say that when i talk to Joe director of nursing he helped me get some help with things that my husband needed. The facility could be cleaner and smell better.
My grandma has stayed in this facility for a month after her hip surgery due to a fall. I will say that this facility is a hit or a miss. But you get different interactions everyday. The reason why I’m giving them a 5stars is because of the wonderful CNA Maria, she went above and beyond everyday for my grandma. Always goes out of her way even though she is busy to provide the extra care my grandma needed. She is truly exceptional. I would also like to add that Maria tries her best understand my grandma as there is a language barrier. physical therapy comes once a day to help & their staff is very helpful and has a great attitude.
CNA told my ill dad to speak “English” lol

I Regret bringing my father here I know it’s alot when a loved one needs to be cared for and it seems like it’s to much to do yourself but If I could do it all over again I would take him home and take care of him myself even as stressful and as hard as it is, because these places don’t care about you or your loved ones let’s be honest they are just showing up for a check and it shows, they don’t come for hours when you call them,,,,, my dad fell on the floor in his room he was there for hours before they helped him up and told me a day later, these places are just to busy chasing the paper trail trying to bill insurance for all the work they are not doing for these people that they are neglecting, it’s really disgusting, I’ve seen animals treated better then how they are treating are vulnerable sick elders I lost my dad there 12-06-2025 he was probably calling for help and they didn’t come help him they called me at 7:30 am and said he was unresponsive and the ambulance is on the way I rushed down there immediately and when I got there five paramedics were in the room with him, they just gave up as I entered the room and said they tried everything for 20 minutes and there’s nothing more they can do, these places are designed to make you die in there neglecting you or put you to sleep, I hope and prey my dad didn’t suffer in his last moments alive…..
Do NOT take your family here!! If you have a senior loved one who cannot fend for themselves or who is frail, do NOT go here, it is Extremely Dangerous! This facility is knowingly neglecting seniors and are not correctly staffed for the care they need and deserve! Our loved one lost over 10 lbs in 4.5 days and we quickly moved them out of there for their safety! This place is Neglecting baths, changing, feeding, brushing teeth, among other things – it is dangerous, not kidding! Do yourself and your family a favor and Look elsewhere and tour the facilities and make sure to stop in at different times to make sure the facilities are taking good care of your loved family and friends! We must advocate for our seniors and never allow them to be neglected or improperly treated! If you or a family member is already at this location and are not being treated properly, find another location and IMMEDIATELY Request and put in for a transfer!! Once you or your family member is out if there safely, put in a complaint to Medicare and anywhere else to report this facility! This facility needs to be reported and we’ve already sent our report to Medicare, this is NOT a safe place for seniors! This place needs to be put out of business or needs new management and accountability immediately. (This has nothing to do with therapy, this is regarding the extreme lack of care for seniors that need assistance)
My mom’s care was good when it comes to Therapy. I found the staff to be consistent in their help to getting my mom more mobile. One time a day is just not enough however. Daily care was hit and miss. Moms help button was out of reach one day. She was left in her diapers too long a couple of times. Food lacked nutrients and she was fed greasy foods and spicy food many times even though they knew she does not have a gall bladder. I never saw her have fresh fruit or fresh vegetables. No bananas apples berries. It looked like canned fruit item and meat was very greasy often. Too many people to staff ratio. I was told 12 to one. Workers looked exhausted and were often moody. Two showers a week is rough on patients. There are some good workers who were friendly encouraging and caring which was helpful. Hit and miss. I was there every day to fill in and visit and was worried everyday wondering what kind of day my mom was going to have. I would say average care. The caseworker was always helpful.
My grandmother has been receiving care at Canyon Vista Post Acute, and I could not be more grateful for the level of compassion, patience, and attentiveness shown by the entire staff. From the moment she arrived, they treated her not just as a resident, but as family. The nurses, CNAs, physical & occupational therapists, and support staff consistently go above and beyond to make sure her needs are met. They take their time with her, listen closely, and respond with genuine care. One thing that truly stood out is how proactive they are even helping coordinate and schedule her follow-up doctor appointments to ensure nothing is missed. A special thank you to Ms. Vivian, who was incredibly efficient and supportive when it came to organizing my grandmother’s outside care. Her communication, dedication, and kindness made a huge difference during a stressful time. Canyon Vista Post Acute is a place where you can trust that your loved one is safe, well cared for, and treated with dignity. I’m sincerely thankful for everything they do for my grandmother and our family. Highly recommended!
I was a patient here from 9/19 to 10/9 and only complaint could be the “FOOD” Great nurses, especially MA RACHEL & CNAs who did so much for me! Liza my night CNA who helped bathe me & kept my linens clean. The Therapy staff helped me achieve my goals & go HOME.
TLDR: They neglected to care for my grandmother who was supposed to be in their rehabilitation program, and her belongings have been missing since she passed away, with this being the last place they were. I wish I could give this place zero stars, my grandmother went here after being briefly hospitalized and she was not eptly cared for after being transfered here after the first day or two. there was no effort to build her strength back up or do any therapy that was promised to us and to her. they just laid her in a hospital bed to the point she had bed sores all over her body, and essentially just waited for her to die. you claim to be a rehabilitation, yet you refuse to do anything more than a five minute task evey couple hours for someone. she went from a walking talking person to by the end of her stay not even being able to sit upright in a chair herself, she then passed away after we brought her back to a hospital when she was wrongfully in my opinion discharged to us very unwell. ontop of all of this the nurses have lost or stolen her belongings. her purse and the contents have been missing for a month. thanks for your lack of care for somebody deeply loved and their belongings.. absolutely disgusting and unreal. I wouldn’t recommend this place to anyone unless they wanna be neglected and stolen from.
Thanking the staff for the care given. The first thing I noticed was there was no urine or feces smell when I came in the door! CNA AND RN Nurses is the reason for my rating. And the ones that went above the call of duty. Thank you Daniel, Wedy, Alexis, Salem, Teresa, Martha, Lupe, Maria, Amour, Fatima. Drs. Regina and Justin listened to my concerns. They kept me clean and pain manageable. And then came my marvelous PT gang, Sasha, Davy Tyler, Samantha, Galen, Serra all worked with the Good Lord to bring me from being non-functional to a fighting chance at walking again. Administration officials listened to my concerns cleared them up right away. I also need to be grateful for every person who prayed for me and supported me in my growth opportunities. And Grace.
I’ve had a terrible overall experience yet there are plenty of good staff such as Selam, Ma Rachel, Katrina, Kat, and Daniel But there are SERIOUS issues. Such as the call light, from the time you push the call light to the time someone comes is a minimum of 30 minutes to over a hour. And once someone actually comes they have a less than happy attitude. Plus they say they’ll get what you need, like when I ask for a blanket they’ll say yes but not come back and I’ll have to remind them multiple times and the odds of getting your request fulfilled is extremely minimal . The food is also poor. Some food is good but comes cold. And they say that you can order what you want but guess what either they get it wrong or they say that they just threw it out. PLEASE DO YOURSELF A FAVOR AND CHOOSE A DIFFERENT REHAB. By the way I’m writing this AFTER the supervisor came by yesterday and “assisted” me in changing my first horrible review to a lot less damaging review. My original review stands, THIS PLACE IS HELL!!!!!
